Recertification and Assurances Form

    This form is used to renew DRE certifications that have not yet expired and those that have not been expired for more than one year.  Recertifications may be submitted up to four months prior to expiration.

DRE Reinstatement Form – 1 to 5 Years

    Use this form to reinstate a DRE whose certification has been expired for one year but not more than five years.

Recertification Checklist – Include a copy of this checklist with all renewal paperwork

Use this form for all certification renewals where the DRE’s certification has not yet expired or has not been expired for more than one year.
